Subacute Care

Subacute care helps residents transition to skilled nursing care or discharges them to a lower level of care. This care may include cardiovascular, pulmonary, respiratory rehabilitation; wound care; IV antibiotic therapy; or pain management. We monitor patient/resident outcomes including such quality measures as healing and medical improvement, functional gains and length of stay.
